MISSION TRIP REGISTRATION FORM

Passport must be valid for a minimum of 6 months from the date of travelling.
Parental consent is required of applicants under the age of 21 years.
All Registration Forms are for official use and will be kept strictly confidential.
Bintan, Indonesia: SGD 180.00 per tripper
*Note:
- The above fee covers cross-border and in-country transportation, accommodations and meals (breakfast, lunch and dinner).
- Kindly note that the above fee takes into consideration any fluctuations in cost, especially that of cross-border transportation (e.g. flight ticket pricing fluctuations depending on how early they are purchased from the travel dates).
- Whatever surplus at the end of the trip will be returned to you, after the full trip accounting has been completed (~3-4 weeks following the team's return to Singapore).
Pre-trip training sessions (mandatory to attend all):
- 28 June 2026 (12 - 3 PM, subject to team leader's adjustment)
- 5 July 2026 (12 - 3 PM, subject to team leader's adjustment)
*Note: The Missions Committee in CMC reserve the right to cancel your registration, should you fail to attend all the training sessions as mandated by our missions policy.
Thank you for your interest in our mission trip. However, as we note that you are unable to participate in the minimum number of training sessions required of each trip member, you may not proceed further with your registration at present.

Thank you for your interest in bringing your child to this mission trip.
Regretfully, we will have to turn your child's registration down. It is unlikely that they may be relatively independent during the trip, and would take up a lot of your own attention and energies, which would otherwise be needed for the responsibilities expected of a team member.
A member of our church's Missions Committee will still contact you to speak with you about this, so kindly await contact if you wish to appeal for your child's registration. Thank you.

As your child/you are under the age of consent, a parent/guardian will need to acknowledge and approve of your child's/your registration.

Terms & Conditions
1. The Applicant must be a church member or a child of church member to be eligible for a subsidy application
- An Applicant who is not a church member (including those below 18 yrs old) but has been attending CMC for at least 6 months, and considers CMC their home church can also be eligible for a trip subsidy if:
a) The Applicant is recommended by their Care Group Leader or Ministry Leader.
b) If the Applicant is not in any care group or serving in ministry, they will need to obtain a recommendation from a Pastor or Church Leader who knows them.
2. The Applicant must also fulfil the following financial criteria to be eligible for a subsidy:
- Children, students and the unemployed are eligible for up to 50% subsidy.
- If currently employed,
a) Applicant with per capita income below $1,250 per month can apply for up to 50% subsidy.
b) Applicant with per capita income below $2,500 per month can apply for up to 25% subsidy.
c) Applicant must provide proof of income (e.g. copy of payslip, CPF statement, etc.) and household size (e.g. copy of household members' NRIC)
3. Approval of subsidy is subjected to the final review and decision by the Missions Committee, and the results will be communicated to you by a member of the Missions Committee.
4. If approved, the subsidy will be granted to you via reimbursement following your successful participation in the mission trip. Therefore, the trip fees will still have to be paid in full first before the trip.
As a member of the church, you may be eligible for a subsidy application according to the terms and conditions stipulated above (kindly read through for clarity).
Please note that the approval of the subsidy is subject to the final review and decision by the Missions Committee, and the results will be communicated to you by a member of the Missions Committee. Thank you.
As you are currently not a member of the church, you may be eligible for a subsidy application only if you have been worshipping with CMC for at least the past 6 months and are able to obtain a recommendation from your Care Group Leader, Ministry Leader, or a Pastor/Church Leader who knows you well.

As you are currently not a regular worshipper of the church, we regret to inform that you are ineligible for a subsidy application.

The church will bear the cost of a standard tier single trip travel insurance policy for mission trippers who are either members of CMC or are regular church worshippers (of a minimum of 6 months) for the duration of the mission trip.
If you already have your own annual travel insurance policy and/or do not need the church to pay for this single trip policy, kindly indicate below.
Kindly note that as you are above the age of 70, your travel insurance coverage will be reduced according to the insurer's stipulations.
The mission trip team leader will inform you of the specifics during the pre-trip training sessions. Thank you.
The church reserves the right not to accept registrants whom the church considers unsuitable to go on the mission trip, at any point from registration to the midst of pre-trip training sessions.
